Assessment of the person's clinical situation and planning of health care provision. Includes:
Assessment of vital signs (Blood pressure, Heart rate, Body temperature, Oxygen saturation,), Blood glucose and Cholesterol
Clinical history collection
Assessment of possible health risk factors
Nursing care plan proposal
Teaching about prevention, treatment of chronic diseases, appropriate procedures according to the person's needs. Advice/referral to specialists, if necessary.
Possibility of online or telephone consultation. Allows remote support and screening of risk factors, overcoming geographical barriers. Enabling more affordable personalized monitoring.
Intramuscular, subcutaneous and intravenous injections. Depending on the drug, with the appropriate medical prescription and evaluation of possible adverse reactions.
Dressings: prevention of infection, wound healing and minimization of skin lesions through the application of specific materials suitable for the tissues present.
Removal of stitches: Assessment of the wound to determine conditions favorable to the removal of surgical stitches, staples, or steri-strips using aseptic technique. And application of a protective dressing.
Suture with steri-strip.
Assessment of the feet (sensitivity tests, protective pressure, reflexes and whether or not there is diabetic neuropathy);
Treatment of diabetic foot (glycemia control, oral therapy monitoring, wound maintenance;
Documentation of the entire process in order to assess progression
Collection of sterile urine for laboratory analysis. Includes the service of inserting a urinary catheter if necessary and delivery of the sample to the laboratory.
Blood collection for laboratory analysis.
Swab collection.

Rapid urine test, which consists of identifying some abnormal components in the urine, possible indicators of urinary tract diseases. For example: urinary infection.

Preparation of medication in compartments divided by days and hours in order to facilitate taking or administration by the caregiver.

Unique provision of hygiene and comfort care with a view to teaching the person or caregiver.

Insertion of a nasal tube into the stomach for feeding and administration of oral medication.

Assessment of surrounding skin, cleaning, disinfection and change of support.

Insertion or removal of a urinary catheter into the urinary tract to drain urine.

Unblocking the urinary catheter with a syringe. Maintaining its permeability

Bowel unblocking by administration of a cleansing enema.

Remove the hardened mass of feces manually. Unblocking the intestine.

Cleansing and assessing the surrounding skin. Applying a new bag if necessary. (in case of colostomy/ileostomy, for example.)

Implementation of oxygen therapy at home, as prescribed by a doctor. Instructions on specific care.

Assistance in the pre-operative phase, preparation for surgery. As well as in the transition phase between hospitalization and return home, with all the adaptations that may imply.

My Personal Journey
I was born in Alcobaça, where I grew up surrounded by family influences linked to work and entrepreneurship. From an early age, I started working in various activities during my adolescence, helping out in my parents' shop and doing odd jobs during the summer. At the age of 21, I moved to Terceira Island, in the Azores, to study Nursing.
This experience further strengthened my connection to nature and solidified me as a person, in an environment rich in traditions, sports and academic life. My experiences include a nomadic life that led me to explore new cultures and challenges, resulting in a strong emotional connection to my roots and a constant search for personal growth.

My Professional Journey
I began my nursing career in the Azores and over the years I have gained extensive experience in various areas, such as community health, geriatrics and psychiatry. I have worked in Portugal and the United Kingdom, where I developed skills in different hospital units. I stand out for the creation of new procedures, such as the implementation of a pre-procedure consultation in interventional radiology.
After experiencing burnout, I reassessed my career and decided to focus on closer relationships with patients, especially in elderly care and mental health. I currently work as an independent nurse, providing home care services and developing innovative projects, such as Ecotherapy, always with the commitment to contribute to positive change in the healthcare sector.
